About This Role
As our Home Care Scheduler, you’ll be more than the keeper of appointments, you’ll be the connector ensuring seniors receive the care, dignity, and support they deserve in the place they call home. What Does Success Look Like As a Home Care Scheduler?
- Coordinate with Care Partners: Manage ongoing home care and home health services with trusted external vendors.
- Own the Schedule: Book all home care and home health appointments in the EMR, making sure everything stays organized and up to date.
- Be the Go-To Contact: Serve as the main point of contact for vendors, building strong relationships and solving issues quickly.
- Ensure Timely Records: Track and receive home care and home health medical records promptly to keep participant charts complete.
- Support Participants: Complete appointment reminders so participants are prepared and supported.
- Collaborate for Seamless Care: Work closely with the Home Care Coordinator and interdisciplinary team to deliver coordinated care.
- Partner on Logistics: Team up with the Transportation Coordinator to ensure participants get where they need to go.
- Adapt and Contribute: Step in with other duties as needed to support the team and participants.
What Does An Ideal Candidate Look Like?
- Passion for Service: Demonstrated commitment to supporting high-risk seniors and frail older adults with compassion, patience, and respect.
- Relevant Experience: Background in coordinating Home Care and/or Home Health services, with direct experience serving frail and elderly adults.
- Technical Skills: Familiarity with healthcare systems and electronic medical records (EMR) preferred.
- Strong Communicator: Excellent communication and interpersonal skills to effectively connect with participants, families, vendors, and care team members.
- Collaborative Mindset: Ability to build trust and foster collaboration across interdisciplinary teams.
- Organized & Proactive: Skilled in managing multiple schedules and priorities with strong attention to detail and problem-solving abilities.
- Certified & Prepared: CPR and First Aid certification required (must be obtained within 60 days of employment—training provided). Alzheimer’s/Dementia care certification is a plus.
Location
- Where You’ll Be: You’ll call our South LA site home base, working alongside a dedicated care team.
- When You’ll Be Here: This role is designed for full onsite presence, five days each week.
- Your Workday Rhythm: A steady Monday through Friday schedule, 8:00 a.m. to 5:00 p.m., keeps your days predictable and balanced.

- Active & On the Move: While most of the work takes place in an office setting, you’ll be regularly engaging with participants, vendors, and team members to keep schedules running smoothly.
