Reports to the Admissions Manager and utilizes clinical expertise to assess patients who may require mental health services. Coordinates patient care across the continuum, ensuring quality, compliance, and effective communication with providers, patients, and families.

Key Responsibilities
Clinical Assessment & Care Coordination

Perform initial psychiatric assessments and obtain authorization when required.

Collaborate with physicians and healthcare providers to ensure effective care plan implementation.

Communicate physician treatment recommendations, including coordination of community services, follow-up care, and patient/family education.

Ensure completeness and accuracy of medical records.

Report patient outcomes to the manager.

Provide Social Work support on inpatient units as needed.
Quality Improvement & Data Management
Patient Registration & Financial Documentation

Conduct patient interviews and gather collateral information for admissions.

Accurately obtain and enter demographic, physician, and insurance information into the hospital system.

Verify and update financial information; pre-certify with insurance carriers to ensure coverage.

Collect and confirm copies of insurance cards and government-issued IDs.

Provide Medicare patients with required documentation (MSP, ABN, etc.).

Refer patients to Patient Account Liaison (PAL) for financial arrangements and assist with additional documentation.

Obtain necessary consents and insurance benefit assignments.
Insurance Verification & Pre-Certification
Admission Coordination

Notify assigned units of patient arrivals.

Prepare charts, ID bands, and labels.

Arrange safe patient escort to assigned units.

Complete mandatory education, annual competencies, and department-specific training on time.

Meet employee health and certification requirements (BCLS/CPR and others as applicable).

Education & Experience
-
Master’s degree in Social Work, Counseling, or related field required.
-
Minimum two years of psychiatric assessment experience preferred.
Knowledge & Skills

In-depth knowledge of psychiatric assessment procedures and targeted patient population needs.

Strong verbal, written, and interpersonal communication skills.
-
Ability to prioritize, organize, and manage workload under pressure.

Proficiency in identifying/utilizing resources effectively.
-
Demonstrated computer literacy and ability to use electronic health record systems.
