About this Role: Under the guidance of the Supportive Care Manager, the Palliative Care Nurse (PCN) is responsible for directing and augmenting care plans tailored to the palliative needs of elderly participants. This role provides ongoing direction and input during the delivery of care plans. The PCN must possess the expertise to assess, plan, and coordinate palliative care in alignment with both geriatric and hospice nursing standards, as well as with Community LIFE protocols.

Schedule: Monday – Friday 7:30 AM – 4:00 PM (occasional other hours due to participant needs)
Location: This position provides services to Community LIFE participants in the Wilkinsburg, Lower Burrell, Tarentum, and Bedford, PA areas. Mileage is reimbursed at the federal rate.
Required Education & Experience: Graduate of an accredited nursing program. Minimum of 2 years of RN experience in palliative or hospice care. Experience teaching patients and other health care workers preferred.
Required Certifications and Licensure: Licensed Registered Nurse in Pennsylvania. Hospice and Palliative Nurse Certification preferred. Valid Driver’s License.
Required Knowledge, Skills and Abilities:
- Knowledge of the medical, social, and emotional needs of a frail, elderly population.
- Comprehensive knowledge and skill within current nursing practice.
- Effective written and oral communication skills.
- Ability to facilitate end-of-life discussion as needed.
- Ability to work with an interdisciplinary team approach to care for the elderly.
- Ability to accept supervision in an appropriate manner.
- Working knowledge of local health care and geriatric service networks.
- Basic working knowledge of Windows operating systems, e-mail, word processing.
