NURSING SUMMARY/ PURPOSE OF POSITION
- The Structural Heart Disease (SHD) Coordinator will function as an integral member of the SHD Service Line. The successful candidate will work in concert with a dedicated team of physicians, nurses, technicians, housestaff and administrators, ensuring optimal care and efficiency for patients with cardiovascular disease. The Coordinator will support program development and strategic initiatives.
QUALIFICATIONS
- Graduate of an accredited school of nursing.
- Current, active licensure as a Licensed Practical Nurse through the Ohio Board of Nursing.
- Maintain current, active BLS certification through the American Heart Association.
- 3 years prior acute care nursing and/or Prior cardiovascular nursing care experience experience preferred.
- Experience with project development and implementation
- Skilled in the use of quality improvement concepts and tools

- Collects comprehensive data pertinent to the patient’s health or the situation.
- Identifies and reports changes in patients’ status to Registered Nurse and/or APRN for evaluation.
- Expected roles include direct patient contact and coordination of care as well as the coordination of weekly meetings and presentations.
- Specific responsibilities will include scheduling outpatient evaluations and follow-ups, implantation scheduling, maintaining the valve database, and overseeing documentation (creation of order sets, etc.) of the patient experience.
- Initiate and coordinate outreach functions
- Actively participates in the Transcatheter Valve Therapy (TVT) and Left Atrial Appendage Occlusion (LAAO) Registries to assure data integrity
- Actively participates in research projects that arise from the Valve Center
- Develops and implements protocols for care of structural heart patients which are evidence based
- Demonstrates use of evidence applied to clinical practice to achieve improved outcomes
- Participates in performance improvement/research activities designed to improve patient care, and/or organizational processes
- Lead monthly Structural Heart conferences
- Coordinate and lead monthly Structural Heart education conferences for medical and nursing staff as requested and needed
- Chair monthly Structural Heart Committee meetings
- Provide follow-up to all departments, staff and physicians for performance improvement

- Participates in and supports hospital safety programs and initiatives.

- Partners with the unit directors to ensure that communication flows in all directions in order to provide clarity, focus, and connection between management, staff, and the medical staff.
- Ensures that staff has all resources necessary to be successful in terms of equipment, processes, information and support.
- Takes the initiative to actively build great teams through communication, coaching, shift activities, and resolving challenges.
- Communicates with peers on a daily basis regarding service delivery and issues that need to be resolved.
- Guides the patient to the next phase of care
- Provides direct patient care when needed.
- Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.
- Participates in Performance Improvement activities of the department as appropriate.
- Management of assigned projects on a daily basis including initiation, follow-through and completion of the project
- Demonstrates use of evidence applied to clinical practice to achieve improved outcomes.
- Serves as an active member of the team, working to ensure the clinical and budgetary efficiency of the department.
- Demonstrates lifelong learning through a knowledge and understanding of best practice and evidence based guidelines for cardiovascular disease management
- The job will require schedule flexibility with regard to shift start and end times

WORKING CONDITIONS:
- Monday-Friday. Occasional weekends and off-shifts to accommodate outreach efforts.
- Subject to frequent interruptions and changes in priority of duties throughout the day.
