Experienced Home Health Speech Therapist – PRN
Service Areas: Prince George’s and Montgomery Counties

Responsibilities:
-
Perform comprehensive speech and language evaluations in patients’ homes.

Develop and implement individualized therapy plans in collaboration with physicians and care teams.

Deliver targeted interventions to enhance communication, swallowing, and cognitive-communication skills.

Educate patients and caregivers on therapy strategies and techniques to support continued progress.

Document all services and patient responses in a timely and accurate manner.

Reassess treatment goals regularly and modify plans as needed to achieve optimal outcomes.

Communicate and coordinate effectively with the interdisciplinary team to ensure quality care.

Maintain compliance with HIPAA, Medicare, and company documentation standards.

Engage in ongoing professional development and stay current on best practices.
Qualifications:
-
Graduate of an accredited Speech-Language Pathology program.
-
Current and valid Speech Therapy license for Maryland.

Minimum of 2 years of clinical experience, preferably in home health or skilled nursing.

Proficient in assessment, treatment planning, and delivering therapeutic interventions to adult and elderly populations.

Familiarity with OASIS documentation is preferred.

Proficient with EMR systems (Kinnser/WellSky or Homecare Homebase preferred).

Strong communication, time management, and problem-solving skills.

Ability to work independently and manage a flexible PRN schedule.

Reliable transportation and a valid driver’s license are required.
