POSITION SUMMARY

Under the direction of a Registered Nurse, this position will work closely with physicians and mid-level providers to focus on quality improvement outcomes for specified patient panels. Duties for this position include: coordinating annual wellness visits, data entry, chart review, and collecting historical information from patients. A strong work ethic, with the ability to multi-task and function independently in a fast-moving work environment are essential functions of this job.

Minimum Education

High school diploma or equivalent.

1 year clinical experience

Required Registration/License/Certification

BLS certification within 90 days of date of hire or transfer.

3 or more years clinic experience with knowledge of disease processes and familiarity with routine medications
