Are you a compassionate nurse with a passion for patient-centered care? We're seeking a dedicated RN or LPN to join our Palliative Care team on a part-time basis
Work Schedule: flexible schedule of three 8-hour shifts per week.
In this role, you"ll provide direct nursing care in alignment with physician established plans of care, working under the guidance of the Palliative Care Medical Director. You'll play a key role in helping patients manage serious illness with dignity and comfort supporting both the patient and their family through clinical expertise and emotional care.
Key Responsibilities:
- Deliver high-quality, compassionate nursing care in accordance with physician orders, state Nurse Practice regulations, and organizational policies.
- Collaborate closely with the provider team to coordinate patient care plans.
- Schedule and manage patient visits and follow-ups.
- Conduct patient outreach and follow-up phone calls to ensure continuity of care and support.
Qualifications:
- Current RN or LPN license in good standing.
- 2 years' experience in geriatrics, long-term care, or a related setting preferred.
- A strong commitment to the values of palliative care, including patient dignity, comfort, and holistic support.
- Excellent communication, time management, and teamwork skills.
