Position Statement
As a member of the North Country Pediatrics care team, the Social Worker provides outreach to patients who are currently experiencing challenges in receiving health care and/or other services to support their health. This position provides the opportunity to meet people where they are in their lives and connect them to services, especially those experiencing social and environmental barriers, mental health challenges, and/or struggling with substance use.
Key Responsibilities: (this should contain no more than 10 bullets)
- Receive referrals from North Country Pediatrics staff to support families with identified risk factors for social determinants of health and connect these families with appropriate community resources, State or federal support programs, for example childcare, transportation, housing, health insurance.
- Understand family systems, community resources, health insurance, healthcare system navigation, and social determinants relevant to improving health outcomes.
- Cooperate closely with the North Country Pediatrics Resource Coordinator and WIC in the Medical Home nutritionist.
- Facilitate collaboration between the Medical Home, North Country Pediatrics, schools, mental health agencies, DCF and other agencies.
- Work directly with families to support their needs as outlined above.
Knowledge, Skills and Abilities:
- Licensed Social Worker preferred.
- 1-3 years of experience working in the health or social services field with understanding of those experiencing poor mental health, struggling with substance use or experience with other underserved communities preferred.
- Familiar with the Vermont system of care plan and the guidelines for implementing an individual care plan for children and adolescents with severe emotional disturbance.
- Knowledgeable in the function of the local Interagency Team, Policy and procedures of DCF regarding children in need of care and supervision and of special education laws.
- Awareness of community resources available to children and families.
- Knowledge of child development and dynamic family issues impacting children at all stages of development.
Pay Information:
$29.13 - $43.77 hourly
