Functioning within the Health System’s mission, values, objectives, policies, and procedures, the Rehab Admission Coordinator serves as a rehabilitation nurse consultant and outreach coordinator with a primary focus on the client’s rehabilitation potential. Responsibilities include performing on-site and/or record reviews; supervising the data collection process; and serving as the liaison case manager with the internal and external case managers. The Rehab Admission Coordinator assumes responsibility for supervising the IRF-PAI data collections and serves as an Educator for patients, families and staff; and has accountability for outreach.
Education: Graduate of an accredited school of nursing with an associate degree in nursing, BSN is preferred.
Licensure/Certification: Valid unrestricted license with the Ohio Board of Nursing
Experience: A minimum of two (2) years of clinical experience specific to the rehabilitation of chronically or catastrophically ill or injured individuals and a minimum of two (2) years recent experience in pre-screening and evaluating patients for admission to post-acute care are required. Strong knowledge of CMS guidelines for inpatient rehab admissions is strongly preferred.
