Reports to the Clinical Nutrition Manager, Provides Medical Nutritional Therapy (MNT), using assessment, goal/intervention setting, planning, implementation, and evaluation. Specializes and/or is certified in Malnutrition, Gastrostomy tubes, Disordered Eating, Heart Failure, Diabetes, Critical Care, Oncology, and more. Educates patients based on their needs. Collaboratively establishes standards of care and protocols for specialized patient populations. Reviews medical records in order to assess nutritional needs and develops nutritional interventions in conjunction with the healthcare team. Evaluates indicators for the aspects of care in the quality improvement process. Serve as a role model, resource, and leader for fellow dietitians, interns, and students. Distinguishes those Dietitians who cannot only deliver direct nutrition care autonomously and optimally but also lead interdisciplinary teams and direct nutrition focused research policies or protocols.

Standards of Care:
- Completes provider and other discipline ordered Consults, nutrition risk screening for assigned units
- Reviews medical records and appropriately assess nutritional status of patients & formulates nutrition plan of care using the following measures and more: Reviewing labs, medications, feeding modality, current medical diagnosis, skin condition, and diagnosing malnutrition
- Performing an NFPE (nutritional-focused physical exam) to determine nutritional status,
- Documenting in the EMR using ADIME (Assessment, Diagnosis, Intervention, Monitoring, and Evaluation).
- Collaborates with the physician and multi-disciplinary team regarding the impact of the nutritional status as it affects total treatment plan.
- Has knowledge of Joint Commission and CMS standards and regulations as it pertains to healthcare and dietetics.
- Selects appropriate feeding route (OG, NG, NJ tubes), mode, and medical enteral/parenteral nutrition formulas
- Maintains appropriate follow-up of patient's nutritional status per protocols and regulations.
- Actively participates in care conferences, discharge planning process and committee meetings assuring follow-up care as appropriate with documentation.
- Actively participates in assigned committees.
- Discusses/interprets appropriate use of pharmacotherapy, makes recommendations as appropriate (supplements, commercial products).
Supervisory Responsibilities:
- Perform human resource functions consistent with job responsibilities as delegated by supervisor: may participate with interviewing, assist with setting up orientation and training, may assist in scheduling, competence assessment and peer review process, provides feedback for staff performance evaluation, mentoring for professional development, and practice advancement.
- Assist in the development and maintenance of EMR clinical nutrition assessment forms
- Assist in updating, ordering, and storage of Nutrition products: tube feeding and ONS formulary
- Responsible for managing the delivery of nutrition and diabetes education and support to both inpatients, outpatients and the community, based on approved standards of care as indicated by Federal, State, accreditation standards and practices of the Academy of Nutrition and Dietetics and the (NSDSME) standards.
- Responsible for ensuring all standards are met for ADCES accreditation for DSMES and MNT education (Coordinator)
- Together with the CNM interviews, hires, trains, evaluates performance, productivity, and clinical competency of personnel. Works in coordination with CNM on colleague relation matters and provides assistance and guidance in problem resolution.
- Assist with other Managerial duties per CNM discretion.
Educator – Inpatient/Outpatient:
- Provides education and counseling (individual and group sessions) in accordance with approved procedures.
- Effectively educates patients/families with appropriate medical nutrition therapy and individualizes recommendations according to learning needs.
- Documents required results of patient education on approved forms.
- Identifies and develops nutrition education tools needed for patients to accommodate cultural framework and educational level.
- Participates in nutrition skill development of EGH employees through educational opportunities. i.e., Nursing-Diabetes Standards and updates.
- Provides orientation/education to new personnel and dietetic interns
- Maintains certification in specialty areas (Diabetes, Eating Disorders).
- Responsible for educating patients on necessary lifestyle changes involved in the provider diagnosis, including nutrition, medication, physical activity, glucose testing, and more.
- Responsible for teaching Cardiac Rehab and Pulmonary Rehab classes.
- Responsible for proper documentation and charting and ensure that all documentation is completed in a timely manner.
- Responsible for managing and collaboration on the coordination of care with other members of the treatment team including, but not limited to: physician, therapist, psychiatrist, SLP, PT/OT
- Works collaboratively with other disciplines inside and outside the hospital system.
Program Development:
- Markets outpatient medical nutrition therapy services to physicians and potential clients.
- Attends and participates in Departmental meetings, RD meetings and CQI/TVM committees and affects change within the clinical team.
- Practices continuous quality improvement and participates in nutritional monitoring studies as needed.
- Identifies opportunities for improvement within department and hospital.
- Implements new concepts in the delivery of nutritional care to patients.
- Participates in departmental goals and objectives.
- Participates in in-service training for staff, students, and physicians.
- Develops protocol or patient care guidelines for area(s) of specialization.
- Coordinates an educational seminar for dietitians, nurses or other professional staff or contributes an article to an in-house or external newsletter to educate professional staff.
- Provides evidence of ability to demonstrate application of at least one new skill learned or approach to work annually.
- Performs competency checks on entry level Dietitians
- Participates in monitoring cost effectiveness of services.
- Responsible for instructing OP classes, Initial, and follow-up for MNT.
- Able to instruct and complete documentation for individual teaching sessions.
- Responsible for completing daily productivity and collecting data.
- Responsible for creating all Outpatient education materials for MNT.
- Responsible for reporting any changes to Marketing web page for Outpatient Nutrition Counseling
- Responsible for marketing Outpatient Nutrition Counseling services to all area Providers
- Responsible for marketing Outpatient program in order to increase patient load/schedule
- Manages development, marketing and scheduling of community events for the purpose of education on Nutrition as a source of revenue and/or community volunteer opportunities.
Professional Growth (Has one or more of the following criteria met):
- Serves as a clinical leader who has earned the trust of peers and managers and has achieved recognition of clinical excellence
- Certified in one or more of the following: CDCES, CEDRD, CNSC, IBCLC, CCTD, CSO, RD-AP, CSP, CSR, CSSD, or CSOWM and/or
- Completed Master’s degree as it relates to the field of Dietetics.
- Credentialed for order writing privileges through Beacon EGH and per policy: ability to add/change diet orders, change tube feedings, order oral nutrition supplements, labs, and micronutrients
- Specializes in Diabetes, Oncology, or other high acuity disease states
- Conducted QPI and Publish Research based on the findings
- Mentors entry level Dietitians, Interns, and students
- Responsible for educating team on changes and progress in specialized area of focus (i.e. Diabetes, Oncology, Cardiology, etc.)
- Develops and participates in revision and update of clinical policies/protocols

Education and Experience:
- The knowledge, skills, and abilities as indicated below are normally acquired through the successful completion of a Bachelor's Degree in Dietetics/Nutrition. Registration Eligible, Registration with CDR (Commission on Dietetic Registration), Membership with the AND (Academy of Nutrition and Dietetics), and Licensed Dietitian in the state of Indiana, and CPR Certification. 2-5 years previous experience required, preferably Outpatient Oncology, Eating Disorders, Diabetes or other high acuity disease state. Clinical Dietitian with nutrition screening, assessment, instruction, and CQI experience is required.
Knowledge & Skills:
- Registered Dietitian by CDR, Licensed Dietitian in the state of Indiana by the medical board required.
- Two to five years of experience as a dietitian in a clinical setting required
- Good verbal and written communication skills.
- A working knowledge of current nutritional standards.
- Master’s Degree related to dietetics field required and/or
- Advanced Certification (i.e. CDCES, CSO, RD-AP, CCTD, CSP, CSR, CEDRD, IBCLC, CSOWM, and CSSD) required
- Credentialed for order writing privileges required upon hire
- Excellent knowledge base/clinical skills in specialty area (Diabetes, Disordered Eating, Oncology, etc).
- Knowledge of hospital organization R/T communication, policies/procedures, standards of care, etc.
- Preparation/experience in education/teaching desired.
- Requires resourcefulness in emergency situations
Working Conditions:
- Works in a hospital office and patient care environment.
Physical Demands:
Requires the physical ability and stamina (i.e. to walk/stand for prolonged periods of time, push carts/wheelchairs up to 50 pounds, to position/lift patients at a maximum of 35 pounds unassisted, over 35 pounds requires assistance, provide CPR, etc.) to perform the essential functions of the position.
