A Brief Overview
Provides clinical nutrition services to inpatient/outpatients. Assesses patient's status, develops and implements an individualized nutrition care plan in accordance with patients' nutrition requirements, the Nutrition Care Process and regulatory standards. Participates in the interdisciplinary team process and conducts training/education programs as required.
What you will do
- • Provides all aspects of Medical Nutrition Therapy to patients using evidence-based research in accordance with the Nutrition Care Process (NCP) and Clinical Nutrition Standards of Practice. * Assesses the nutrition status of patient as it relates to their diagnosis and medical condition, utilizing the medical record, anthropometric data, visual assessment and other subjective data. * Develops and implements a plan of care to meet identified patient problems or needs, writes measurable expected outcomes and plans according to the patient's progress. * Documents evaluation of patient care outcomes through direct interview, calculating nutrient intake and monitoring tolerance of nutrition therapy intervention(s). * Demonstrates knowledge, technical, interpersonal and critical thinking skills necessary to provide care, maintain clinical competency and meet age-specific patient requirements/standards. * Provides follow up with patients and family members as appropriate regarding patient’s nutrition goals, and individualized plan of care. * Assists in implementing standards for exceptional patient-centered care and instruction, as well as methods of evaluating care and instruction. * Works consistently within patient care policies and procedures, demonstrated competencies, with consideration given to risk management, infection control and patient safety practices. * Participates in interdisciplinary team conferences to ensure optimal patient care. * Assesses learning needs of patient/family and provides necessary education as indicated. * Collaborates with other healthcare team members in planning, implementing and evaluating nutrition care plans. * Documents all pertinent information in the medical record clearly, concisely and within the appropriate time frames.
- • Acts as a liaison between the patient, nursing and medical staff to answer questions, provide support and assistance to resolve concerns relating to clinical nutritional services.
- • Maintains patient medical records; performs charting duties; records activities and observations. Utilizes electronic health records and other media to enter and retrieve data.
- • Maintains appropriate documentation to comply with third party billing standards.
- • Confers and works collaboratively with patient/family/clinic/hospital staff regarding all aspects of patient needs in a customer responsive and courteous manner.
- • Complies with the Premier Expectations standards and the promotion of patient/customer satisfaction.
- • Maintains professional credentials and education necessary to keep abreast and maintain a current knowledge of clinical and theoretical principles, concepts, and techniques. Attends continuing education, staff development, and mandatory programs.
- * Maintains a clean, orderly, safe environment for patients and personnel. Adheres to established policies, compliance/quality improvement, patient safety, environment and infection control standards.
Qualifications
- Bachelor's Degree From an accredited school with a major in food and nutrition and successful completion of a dietetic internship, traineeship or coordinated program approved by the Academy of Nutrition and Dietetics. Required
- Master's Degree A Master’s degree is required for all newly credentialed practitioners as of January 2024.
- Qualified by education, training or experience to work with the neonate, infant, adolescent, adult and/or geriatric patient population as specialty assignment dictates.
- REG DIET - Dietitian – Registered Registered by the Commission of Dietetic Registration (CDR) of the Academy of Nutrition and Dietetics Upon Hire Required
- Licensed by the State of Tennessee Board of Dietitian/Nutritionist Examiners. For new residents: Must obtain a state license within 180 Days Required
- Minimum 2 years experience With a Bachelor's degree in varied clinical settings: including but not limited to intensive care, step-down, cardiology, med/surgical units, and patient education – individuals and groups. Required
- Minimum 4 years experience Varied clinical settings. Preferred
- No experience for all new graduates with a Master’s degree that are newly credentialed practitioners. Required
