If so, we have an exciting opportunity for you to work as a Care Coordinator for Kalamazoo County’s Crisis Stabilization Unit!
What You Will Be Doing
As a Care Coordinator for the Crisis Stabilization Unit (CSU), you’ll collaborate with CSU staff and Primary Clinicians to develop individualized discharge plans for individuals who have experienced an acute crisis and may have high level of care needs. You’ll provide back-up to the Hospital Liaison for Adults with Mental Illness and/or a Developmental Disability, completing length of stay reviews and coordinating individual discharge plans. You’ll assist the Specialized Residential Team by completing initial documentation such as client assessments and Individualized Plans of Service and conducting record reviews to establish clinical eligibility for services and ensure Home and Community Based Services (HCBS) rules and regulations are being met and followed. In addition, you will monitor and analyze client data and outcomes to identify patterns, health risks, and utilize this data to develop a comprehensive plan of care for individuals served.
What We Require
You will be a Master’s level licensed professional with a minimum of two years’ experience in behavioral health care services working with adults. You must also have one of the following Michigan Licensures: Licensed Master Social Worker (LMSW) or Licensed Professional Counselor (LPC). Supervision for those with a limited license must be approved by the ISK Credentialing Committee. A valid Michigan Driver’s License is also required.
You will have a strong understanding of the needs of persons with mental illness, intellectual/developmental disabilities, and substance use disorders. You will understand how to implement a strong recovery philosophy and be able to collaborate and develop teams.

Physical Requirements/Working Conditions:
Physical Efforts – Job demands include prolonged sitting and standing as appropriate. May occasionally require light lifting up to 25 pounds, stooping, kneeling, crouching, or bending as appropriate. Requires coordination of hands and/or eye/hand/foot.
Working Conditions – Office environment with noise from computers, copy machine, and telephones. Use of computer screen for periods in excess of 30 minutes at a time. Possible eyestrain from extended periods of viewing computer screen. May be exposed to bloodborne pathogens, infectious diseases, and parasites. Travel throughout the Kalamazoo area is required.
