We are seeking dedicated and compassionate Social Field Assessment personnel to join our team. This role involves conducting assessments in the community to evaluate the needs of individuals and families. The ideal candidate will possess strong interpersonal skills and a commitment to supporting clients through various challenges, ensuring they receive the necessary resources and support.
Job Description:
- Conduct comprehensive field assessments to evaluate client needs and circumstances.
- Travel to members’ homes or community locations to complete social risk assessments.
- Evaluate and document social determinants of health, including housing stability, food insecurity, transportation access, social support, financial concerns, and safety risks.
- Assess and document client progress, ensuring all information is accurately recorded.
- Build rapport with members, demonstrating cultural sensitivity and strong communication skills.
- Utilize standardized tools and electronic systems to collect and accurately record assessment data.
- Identify urgent social or environmental needs and escalate appropriately to care managers or supervisors.
- Utilize motivational interviewing techniques to engage clients effectively.
- Coordinate follow-up as necessary to ensure members are connected to needed services.
- Maintain documentation in compliance with HIPAA, organizational policies, and health plan requirements.
- Maintain confidentiality and adhere to ethical standards in all interactions.
- Attend ongoing training and quality assurance reviews.

Qualifications/Certification:
- Bachelor’s degree in Social Work, Public Health, Human Services, Nursing, or a related field (required).
- Familiarity with Medicaid, Medicare, or managed care systems is highly desirable.
- Working knowledge of social determinants of health and community resource navigation.
- Valid driver’s license, proof of auto insurance, reliable transportation, and willingness to travel locally (required).
- Comfortable using digital tools, tablets, and electronic documentation systems.
- CPR/BLS
