Reports to the Residency Program Director. Assists in the development and maintenance of pharmacy services that promote and provide pharmaceutical care. Participates directly in clinically-oriented services. Performs the Essential Functions outlined in the job description for a pharmacy resident.

Participates in drug regimen management and performs clinical services by:
- Changing the duration of treatment for a current drug therapy
- Adjusting the strength, dosage form, frequency of administration or route of administration of a drug.
- Discontinuing the use of a drug.
- Adding a drug to the treatment regimen.
- Recommending, ordering and monitoring laboratory tests.
- Participating as an active member of interdisciplinary patient care teams as appropriate.
Serves as a resource to the Medical Staff and other healthcare professionals concerning pharmaceutical care by:
- Providing drug information services.
- Reviewing patient medical records and making recommendations about appropriate drug therapy.
- Identifying problems and making recommendations concerning patient compliance, therapeutic response, drug interactions and similar issues.
- Collecting and analyzing data.
Serves as a resource to the Pharmacy Services staff concerning clinically-oriented issues or questions by:
- Creating and presenting in-services, continuing education, and other educational topics as appropriate.
- Writing department-wide newsletters.
Assists with the Pharmacy Services department's participation in research and clinical drug trials by:
- Completing a longitudinal year-long research project with appropriate preceptor oversight
- Maintaining records (etc.) as required
Assists with drug usage evaluation and adverse drug reaction reporting by:
- Recommending areas to be studied
- Preparing or assisting with the preparation of criteria for review.
- Collecting and analyzing data.
- Preparing reports for presentation at various hospital committees.
- Other activities as required by rotation preceptors
Assists with management of pharmaceutical formulary and cost containment by:
- Make formulary presentations, in coordination with pharmacy leadership, regarding new agents and place on formulary.
- Review new agents and, in coordination with pharmacy leadership, develop guidelines for use.
- Complete formulary management reviews and/or medication use evaluations as requested by preceptors or pharmacy management.
Performs other functions to maintain personal competence and contribute to the overall effectiveness of the department by:
- Completing other job-related duties and projects as assigned.

- Attends and participates in department meetings and is accountable for all information shared.
- Completes mandatory education, annual competencies and department specific education within established timeframes.
- Completes annual employee health requirements within established timeframes.
- Maintains license/certification, registration in good standing throughout fiscal year.
- Direct patient care providers are required to maintain current BCLS (CPR) and other certifications as required by position/department.
- Consistently utilizes appropriate universal precautions, protective equipment, and ergonomic techniques to protect patient and self.
- Adheres to regulatory agency requirements, survey process and compliance.
- Complies with established organization and department policies.
- Available to work overtime in addition to working additional or other shifts and schedules when required.

Education and Experience
- The knowledge, skills and abilities as indicated below are normally acquired through the successful completion of a Bachelor's Degree in Pharmacy, a Master's Degree in Pharmacy or a Pharm D Degree from an ACPE-accredited college or university. A license or demonstrated eligibility for licensure, to practice pharmacy in Indiana is required. Preferred candidates will have a well-rounded curriculum vitae and are a highly motivated pharmacist wanting advanced education and training.

- Demonstrates thorough knowledge of policies, procedures and standards for the provision of pharmaceutical services, drug therapy and current treatment modalities.

- Demonstrates the interpersonal skills necessary to interact effectively with others, in order to ensure the provision of pharmaceutical services.
- Demonstrates good communication skills, both verbal and written. Must be able to communicate in a tactful, sincere, effective manner with patients, families, Physicians, other healthcare professionals and various Hospital staff. Also, must be able to make effective presentations and communicate appropriate information to patients, staff members, medical staff and other Hospital personnel.
Working Conditions
- Works in an office environment and/or patient care area.
- Occasional exposure to potentially hazardous substances and/or infectious diseases.
- The resident must also be willing and able to be mobile across the Memorial Hospital of South Bend campus.
- Extensive use of keyboarding on computers.
- Must continually change priority based on patient need or project-based learning requirements.
Physical Demands
- Requires the physical ability and stamina to perform the essential functions of the position.
