Licensed Social Worker
In collaboration with the patient/family, case managers, physicians and interdisciplinary team, this role provides comprehensive assessments (ASAM), complex discharge planning and provides therapy for patients. The social worker assists patients and families to identify the psychosocial barriers that impact the discharge planning process. The Social Worker provides assessment, advocacy, education and information, referral and linkage, coordination, counselling, and crisis intervention to assigned patients. Services are provided in the inpatient settings. This role works is in conjunction with the multi-disciplinary team to develop a discharge plan that is safe and effective for the patient. The role is responsible for the implementation and coordination of the transition of care needs under the direction of the System Director of Behavioral Health and Center for Substance Recovery.

Minimum Education:
- Master’s Degree in Social Work
Minimum Work Experience:
- Health care or related field
Knowledge and Skills:
- Ability to create and analyze data, perform gap analysis. Proficient in Excel, other Microsoft Office products
- Outstanding oral and written communication skills
- Ability to work independently in accordance with policy and procedure.
- Ability to thrive in an unstructured environment
Required Licenses, Certificates, Registrations:
- Licensed Social Worker in the State of Illinois
