Job Summary:
The Patient Financial Counselor will provide vital support to patients and their families by assisting with the Medicaid and VT Health Connect and NY Marketplace (Exchange plans) application process. This role will involve educating patients about their financial options, guiding them through the Medicaid eligibility criteria, and ensuring that all necessary documentation is completed accurately. The counselor will collaborate with healthcare providers, case managers, and external agencies to facilitate access to Medicaid benefits and enhance the financial wellbeing of patients.
Key Responsibilities:
- Patient Outreach: Engage with patients to assess their financial needs and explain the Medicaid application process, including eligibility criteria and required documentation.
- Application Assistance: Assist patients in completing Medicaid applications, ensuring accuracy and compliance with all regulations and guidelines.
- Documentation Management: Gather and organize necessary documents for the Medicaid application, such as income verification, assets, and residency proof, to streamline the application process.
- Follow-Up Support: Monitor the status of Medicaid applications and provide ongoing support to patients throughout the review process, addressing any issues or additional requirements as they arise.
- Financial Education: Educate patients and their families about available financial assistance programs, including Medicaid, charity care, and other options to help them manage their healthcare costs.
- Collaboration: Work closely with clinical staff, social workers, and external agencies to ensure a comprehensive approach to patient financial assistance and support.
- Compliance Adherence: Maintain knowledge of federal and state Medicaid regulations to ensure compliance in all processes related to the application and counseling services.
- Data Entry and Reporting: Accurately enter patient information and application status into financial systems, generating reports as needed to track Medicaid applications and outcomes.
