Summary
Working as a key member of the multidisciplinary team, the Care Manager ensures the effective delivery of patient care through a collaborative approach. The Care Manager interacts with all members of the care team on a daily basis to facilitate optimal discharge planning and care coordination. Additional duties include performing admission, outpatient, pre-admission, and continued stay utilization reviews, completing referrals to community resources and gathering and communicating relevant information to members of the care team including patients, physicians, and payors.
Minimum Qualifications
Current licensure as a Registered Nurse in Pennsylvania.
Two years of work experience as a Registered Nurse in an acute care setting.
Highly effective communication skills.
Strong negotiation skills and the ability to identify and address complex clinical and social issues with patient/family and members of the multidisciplinary care team.
Ability to navigate electronic medical records and web based applications.
Preferred Qualifications
Bachelor’s degree in nursing or a health-related field.
Five years of work experience as a Registered Nurse in an acute care setting.
Certification in Case Management.
Experience in care management utilization, and/or post-acute care delivery.
