JOB SUMMARY
The primary emphasis of the RN Case Manager is to partner with all other members of the health care team to assure efficient care of inpatients, avoiding delays and denials whenever possible. The Case Manager, working with health care team, strives to achieve the best outcomes of appropriate utilization and safe, efficient and timely transition and discharge planning functions. Required to work weekends.
REQUIREMENTS
- Required Associate's Degree
- 2-4 years Related experience and/or training or equivalent combination of educational experience UM/CM/Discharge Planning experience preferred. Required
- CCM - Certified Case Manager
- CPR - AHA Healthcare Provider
SPECIFIC REQUIREMENTS
