Position Purpose: Leverages personal drive and collaborative energy to build relationships with organizations and individuals who may refer patients to Schwab Rehabilitation Hospital. Utilizes strong multitasking skills to efficiently conduct onsite assessments at referring hospitals to determine patient suitability for rehabilitation. Collaborates with referral sources and Schwab’s Admitting Department to ensure timely patient admissions. Supports Schwab in meeting its budgeted inpatient census goals. This position reports to the Schwab Inpatient Access Manager. Key Job Activities: • Establishes and maintains strong relationships with organizations and individuals who may refer patients to Schwab Rehabilitations Hospital through face-to-face meetings. These contracts include discharge planners, social workers, case managers, nurses, therapists and physicians • Education of patients, family members, and referral sources on acute rehabilitation. • Collaborates effectively with appropriate hospital staff to serve existing referral sources while continually striving to increase referral volume. • Actively seeks to identify new referral sources by scheduling a designated number of quality appointments per day and making regular contacts each week. • Demonstrates strong ability to multi-task in conducting analysis, decision-making, and time management when reviewing referrals for admission. • Helps coordinate and schedule informational presentations for referring individuals and organizations. • Performs on-site and/or record review including telephonic pre-admission screening and bedside assessment and responds to patients, families and hospital referral source with a tentative 24-hour admission commitment. Responsive and attentive to needs of the referral source. • Completes the pre-admission screen when appropriate and assures that appropriate physicians review is completed prior to admission. • Coordination of smooth transition from the referring facility to Schwab Rehabilitation Hospital. This includes ensuring patient is medically stable and updated medical records are provided during admission. • Provides recommendations to appropriate level of care for candidates who are denied admission. • Completes all required pre-admission screening documentation on time and accurately to determine that services are appropriate, reasonable, and necessary. Ensures physician has signed off before admission. sinaichicago.org • Works closely with the Access Manager to enhance customer service to referral sources through innovative and dynamic approaches. • Dynamic team member that works with other clinical liaison to ensure volume goals are achieved at Schwab Rehabilitation Hospital. • Demonstrates excellent clinical skills and understanding of the insurance and managed care qualifications for acute and sub-acute rehabilitation in the review, preparation, and coordination of referrals. • Customer Service Responsibilities: o Demonstrates excellent customer service and standards of behaviors as well as encourages, coaches, and monitors the same in team members. This individual should consistently promote teamwork and direct communication with co-workers and deal discretely and sensitively with confidential information. • Responsibilities in Quality Improvement: o Contributes by identifying problems and seeking solutions. Promotes patient/family satisfaction where possible; participates in departmental efforts to monitor and report customer service. • Performs other duties as assigned. Education and Work Experience: • Degree in nursing, social work, physical therapy, occupational therapy, athletic trainer, or speech therapy. • Experience as a liaison or working autonomously. • Experience in post-acute care.
