This individual will be expected to monitor a specific group of patient’s care and support them in achieving short/long term health goals. The Care Manager will coordinate with providers to ensure patients’ needs are met. A working knowledge of the clinical field is desired for this position.
Chronic Care Management (CCM)
CCM stands for Chronic Care Management, which is a healthcare delivery model aimed at providing comprehensive and coordinated care to patients with chronic conditions. CCM services are designed to improve the quality of care, enhance patient outcomes, and reduce healthcare costs by addressing the complex needs of individuals living with chronic illnesses.
CCM Manager Responsibilities:
- Leadership & Strategy: Develop and implement comprehensive CCM strategies that align with organizational goals and enhance customer engagement.
- Technology Management: Oversee the deployment and management of CCM software and tools, ensuring they meet the needs of our customers and business.
- Team Management: Lead and mentor a team of CCM specialists, providing guidance and support to ensure high performance and professional development.
- Process Optimization: Analyze and improve communication processes to increase efficiency, effectiveness, and patient satisfaction.
- Collaboration: Work closely with cross-functional teams, including Marketing, IT, and Quality assurance, to ensure seamless integration of CCM strategies.
- Analytics & Reporting: Monitor and report on key performance indicators (KPIs) related to patient communication, providing insights and recommendations for continuous improvement.
- Patient Insights: Gather and analyze patient feedback to understand their needs and preferences, using this information to enhance communication strategies.
- Compliance: Ensure all communication practices adhere to legal and regulatory requirements, including data protection and privacy laws.
Manager Qualifications:
- Three years of previous experience in healthcare.
- One year of experience in a managerial or supervisory role.
- Strong leadership and team management skills.
- Excellent organizational and problem-solving abilities.
- Ability to work collaboratively with diverse teams and departments.
- Knowledge of healthcare regulations and compliance standards.
- Strong communication and interpersonal skills.
- Proficiency in electronic health records (EHR), Microsoft products, and other relevant software.
- Have Obtained a Medical Assisting Diploma or Certification by an accredited source.
