Health Information Representative

This position is responsible for all the information that the patient provides concerning his or her symptoms and medical history, the results of examinations, reports of x rays and laboratory tests, diagnoses, and treatment plans as well as assisting in the front office areas.
Responsibilities:
1. Responsible for accurately inputting diagnostic test/procedure results in Electronic Health Records (EHR) application within established timeframes.
2. Accurately scans health information into patient’s EHR by using the Four Point Check process for verification (Patient name, Date of Birth, Location/Provider and Date of Service).
3. Responsible for ensuring authorizations for release of patient information are complete, logged and processed ensuring minimum necessary requirements are followed and completed timely per policy.
4. Reviews Provider schedules for upcoming appointments to determine if all patient’s results/reports have been received, follow-up as appropriate.
5. Performs Medical Records Quality Improvement audits, as necessary.
6. Reports Duplicate Medical Records according to procedure.
7. Reviews system to verify/process site lab results that failed to post to patient record.
8. Sort/distribute incoming mail and stamp outgoing mail.
9. Answers phones and assists callers in a pleasant and professional manner.
10. Performs all other related duties as assigned.

1. Excellent customer service skills, ability to resolve basic customer service issues and/or escalate to supervisor as needed.
2. Ability to read, write and speak in a clear and professional manner.
3. Ability to complete basic math computations.
4. Proficient in beginning/intermediate computer skills.
5. Excellent follow through skills and attention to detail.
6. Ability to read and understand medical terminology.
7. Ability to multi-task and work in a fast-paced environment.
8. Ability to function as an effective member of the team.
9. Knowledge of HIPAA Guidelines including the Protected Health Information (PHI) privacy requirements.
Qualifications:
1. High school Graduate or certificate from healthcare-related training program required (i.e. Medical Assistant, Medical Management, Administrative Assistant).
2. A minimum of two years experience in a medical records function required; experience with electronic health records preferred.
3. Bilingual: English/Spanish/Mandarin/Cantonese depending on location preferred.
